A Principal Guide
A useful teacher observation form starts with what matters most in your classrooms. With Feedback Flock, you can create a custom form that reflects your school’s instructional priorities and gives your team a consistent foundation for meaningful feedback. Follow the steps below to build your form for classroom walkthroughs, formal observations, focused coaching conversations, goal setting forms, and many others.
Prerequisites: You need Principal or District Admin Role.
Create Your Custom Form
Before building your custom observation form, work with your instructional leadership team to clarify what you want to observe and how it connects to your curriculum, teaching framework, and schoolwide goals. Identify the specific, observable teaching practices you want to focus on and agree on what success looks like. This shared understanding will help you create a focused form, support consistency across observers, and give teachers clear, actionable feedback.
When you’re ready to build your first form, follow along below:
- Access the ‘Forms’ page
- Select ‘+ New Form’.
- Make a choice between Observation Form or Self Evaluation Form.
Note: Observation Forms are completed by an Observer and use cases include Form and Informal Observations, Lesson Planning, Walkthroughs and Pop-ins; whereas the Self Evaluation Form is completed by the Teacher and use cases include beginning of year goal setting and end of year self reflection. - Select ‘Campuses’ if this is a shared, school-wide form or select ‘One person only’ if this Observation form should be limited to a specific Observer.
- For ‘Campuses’ select your school to assign the form.
For ‘One person only’ select the individual who should be able to make the Observations for this form. - Rename the form from by clicking the ‘Pencil icon’ next to ‘New Observation Form’.
Note: We suggest avoiding using the year in the form name as you should use the same form year over year to acquire historical growth trends and comparison analytics. - Now its time to build your fields.
Note: that Observer and Teacher Name, as well as their emails, and the date and time of the form start are recorded by the form, so these are not necessary fields to create. - Start with a ‘Section’ field to help group form questions together.
To do so, drag a ‘Section’ from the Field Library to the ‘+ Your Form is Empty’ area. - Rename the Section Title from ‘Section Header’ to a relevant name like ‘Student Engagement’
- Next creating your relevant Observation input questions using the Short Text, Long Text, Dropdown, Radio Group or Rating Fields.
For example you can use a ‘Rating’ field to propose ‘Students are engaged in the lesson’.
Add its helper text along the lines of, “1 = Zero Students Engaged, 5 = All Students Engaged”
Then set the required MIN rating from “1” to MAX rating of “5”. Note: Choose your own values, but for best practices keep the minimum and maximum consistent throughout the form. - Each input field automatically includes ‘Quick Notes’ where the observer can supply more detailed feedback.
- Use the ‘Preview’ button to ensure the form appears as you expected and make any necessary changes before processing.
- When you complete your last field, click out of the field and the form will autosave.
- We suggest using the ‘Preview’ button again and reviewing the form in its entireity.
